An AI Chatbot Governance Checklist for Medical Clinics

An effective AI chatbot governance checklist for medical clinics defines what the chatbot may do, what it must not do, when a human must take over, and how the clinic will review performance. Before connecting an automated assistant to WhatsApp, Instagram or Messenger, write down its approved tasks, escalation rules, privacy practices and accountable owner.

For many clinics, the first use case is administrative: answering routine questions, sharing opening hours, or helping patients book and cancel appointments. The risk changes when a conversation moves toward symptoms, diagnosis, treatment, medication or an urgent concern. A governance plan makes those boundaries explicit before the chatbot meets a difficult message.

If you are still deciding whether your clinic needs a chatbot at all, start with our guide to AI chatbots for private medical clinics, then come back to this checklist before you go live.

Start with a narrow, documented purpose

Avoid launching a chatbot with a vague instruction to "answer patients." Choose specific tasks the clinic can verify, such as explaining opening hours, providing location information, or showing available appointment slots from the clinic's real schedule.

Document the intended users, supported languages, channels, operating hours and tasks. List what the system is not designed to do. An administrative assistant should not diagnose a condition, recommend treatment, or reassure a patient that a symptom is harmless.

A narrow first release is easier to test and maintain. Once the clinic understands the common questions, failure modes and staff workload, it can decide whether to expand the scope. Each new capability should get its own review rather than being switched on simply because the technology can generate a response.

Write a short purpose statement that staff can remember, for example: "The assistant provides approved administrative information and helps with scheduling; clinical questions go to qualified staff." Use it to guide scripts, staff training and vendor discussions. If a proposed feature does not fit the statement, review it before activation.

Define escalation rules before launch

The chatbot needs a reliable path to a human. Write examples of conversations that require handoff: requests the system does not understand, complaints, sensitive account or billing questions, and messages asking for clinical advice. Decide who receives each handoff and what happens outside business hours.

For potentially urgent or emergency concerns, the system should not try to assess severity as if it were a clinician. The clinic's approved wording should direct the person to appropriate urgent or emergency services according to local guidance. Have a qualified professional review these messages before deployment.

A practical escalation matrix helps:

Conversation typeChatbot roleRequired next step
Opening hours or locationProvide approved informationNo handoff unless unclear
Appointment bookingOffer genuinely available slots and record the requestConfirm the booking accurately: doctor, date, time, branch
Cancellation or reschedulingFollow the approved scheduling policyShow the updated status or hand the request to staff
Clinical questionAvoid diagnosis or treatment adviceRoute to qualified clinic staff
Urgent or emergency concernDo not triage or delay careShow approved emergency guidance and alert staff
Complaint or unclear requestAcknowledge its limits without arguingOffer human support

Test the handoff itself. A message that says "someone will contact you" is useless if no one owns the queue. Assign a responsible role, set an internal response target that suits the clinic, and audit whether handoffs are actually closed. Include a fallback for when the assigned staff member is unavailable.

Protect privacy and limit data collection

Before launch, map what information the chatbot requests, where it is stored, who can access it and how long it is kept. Collect only what the defined administrative task needs. If a booking can be completed without a detailed medical history, do not ask for one just because the chat window allows it.

Do not assume a consumer messaging channel suits every kind of health information. Make the clinic's communication policy clear to staff and patients, and direct sensitive clinical exchanges to the appropriate channel or process. Check the privacy and data-protection obligations in each country where the clinic operates.

Review user permissions. Reception staff may need booking details but not clinical notes; clinicians need different access from marketing staff. Use role-based access, review who has access regularly, and remove it when someone changes role or leaves. See clinic staff permissions and patient data for a fuller guide.

Ask vendors clear questions about encryption, hosting, subprocessors, data export, deletion, backups, incident response, and whether customer data is used to train models. Get written answers and have the appropriate legal, security or compliance professional assess them. Do not describe a system as compliant or certified unless that status has been independently verified for the relevant product and jurisdiction.

Create a simple incident process. Staff should know how to report a message sent to the wrong person, unauthorized access, or a suspected data exposure. The process should name who assesses the issue, who contacts the vendor, and who decides whether a notification is required. Requirements differ by jurisdiction, so get professional advice rather than relying on a generic checklist alone.

Keep a human accountable for patient-facing decisions

Automation can answer repeat questions and move routine bookings forward, but the clinic remains responsible for its service. Assign a named owner for the chatbot's configuration, approved content, staff training and incident review.

The owner keeps a simple change log: what changed, why, who approved it and when it went live. If fees, hours or appointment availability change, update the source information too. Outdated content causes operational problems even when the wording sounds convincing.

Show staff how to correct an inaccurate answer, pause a workflow, or report a concerning interaction. The goal is not to defend the chatbot: if the assistant gets something wrong, the team resolves the patient's issue first and investigates the cause second.

Be clear about who can switch off the assistant or one of its features. If the booking connection stops reflecting current availability, for example, the clinic should be able to stop automated booking and fall back to a manual process. Walk through the pause procedure with the staff who will use it; a written policy is useless if no one knows where the switch is or what to do next.

Test realistic conversations, not just ideal prompts

A chatbot may perform well on clean examples and then struggle with spelling errors, dialect, mixed-language messages, incomplete information, or a patient who changes the subject. Build a test set from common administrative questions and realistic edge cases, using fictional data rather than real patient details.

At a minimum, the test set should cover:

  • Local dialects: an Egyptian colloquial request for a check-up "tomorrow morning", and a Gulf-dialect request for a same-day appointment.
  • Arabic–English mixing: "ممكن appointment يوم Sunday؟", or Arabic typed in Latin letters.
  • Typos and incomplete messages: a misspelled doctor's name, a date with no month, a one-word message.
  • A medical question in the middle of a booking: "Book me for tomorrow, and should I stop my blood-pressure tablets before?" The expected result: the booking continues, and the medication question goes to staff with no medical opinion.
  • An emergency message: severe chest pain or difficulty breathing. Expected: immediate direction to emergency services and a staff alert, not a list of slots.
  • A reschedule request: does the assistant follow the approved policy, or hand the request to a person if it isn't allowed to change bookings?
  • Out-of-hours messages: what does the assistant say when it hands over at night, and when will staff actually reply?
  • A direct request to speak to a person, an angry complaint, and a price question about a service the clinic doesn't offer.

For each case, record the expected reply, the actual reply, pass or fail, and the action needed. Re-run the same set after every significant change to content or settings so you know a fix didn't break something else.

Monitor quality after deployment

Going live is not the end of the work. Set aside a short weekly slot for the owner to review:

  • A sample of routine conversations: are replies accurate, polite, brief and in line with approved information?
  • Every escalated conversation: why was it handed over, did someone reply, and how long did it take?
  • Wrong or duplicate bookings: the wrong doctor, the wrong branch, or the same patient registered twice.
  • Complaints and feedback that mention the assistant.

Track a small set of measures: the share of handoffs actually closed, the average staff response time to escalations, the number of bookings that needed manual correction, and the questions the assistant repeatedly fails. Those repeated failures are the best source for updating approved content.

Record each decision in the change log, and review performance monthly with the responsible doctor or clinic manager: is the scope still right, is a new escalation rule needed, should a feature be paused for now?

How Ehgizli fits into a clinic's workflow

Ehgizli's booking assistant works on WhatsApp, Instagram and Messenger and is included in the Professional and Growth plans. It is built with fixed rules that match this checklist:

  • No medical advice: it never gives diagnoses or treatment suggestions.
  • Emergencies: it tells the patient to call emergency services immediately and escalates the conversation to staff.
  • The patient's language: it replies in Arabic or English, matching the patient.
  • Real availability: it checks the clinic's actual schedule before offering any time, and shows at most five slots at once.
  • New patients: it creates a patient record after collecting name, phone number and date of birth.
  • Cancellations: only when the appointment is at least two hours away.
  • No rescheduling: requests to move an appointment are handled by a person.

When a patient asks for a person, or the assistant can't help, the conversation moves to staff, the AI stops replying in it, and a doctor or admin at the clinic receives an in-app alert. In the Conversations screen, staff can switch AI replies on or off for each conversation. Admins and front-desk staff never see clinical records.

These rules don't replace the clinic's own governance: you still need an owner, a test set and a regular review. For setup steps, see how to automate clinic appointments via WhatsApp and how chatbots improve appointment booking; the full feature list is on the features page.

Frequently asked questions

Can a clinic chatbot provide medical advice?

An administrative chatbot should not diagnose or prescribe unless a separately validated and properly governed clinical use case has been established. Define the chatbot's scope and route clinical questions to qualified professionals.

Who should own chatbot governance?

A named person at the clinic, drawing on clinical, operational, privacy and technical expertise as needed. Responsibility should not be left to the vendor or the front desk alone.

What should happen when a chatbot is unsure?

It should not invent an answer. It should explain its limits in plain language and offer the approved route to a person.

How often should a clinic test the chatbot?

Before launch, after material changes, and periodically using real-world patterns with privacy safeguards. Review failures and handoffs as part of routine quality checks.

Does using a cloud platform guarantee compliance?

No. Requirements depend on the jurisdiction, configuration, contracts and the clinic's own practices. Get qualified advice and verify the specific controls and obligations that apply to you.

Put governance in place before scaling automation

A chatbot is most useful when patients know what it can do and staff know what happens when it can't. Document the scope, test escalation, protect data, assign ownership and monitor performance. Start with routine administrative tasks and expand only when the clinic can carry the extra responsibility.
